Please call our office during business hours to schedule an appointment. Our new patient paperwork packet is located below for your convenience. We are happy to mail to you if you would prefer. Please complete the paperwork prior to your arrival of your appointment time. We like for our patients to arrive 15 minutes early so that we can stay on schedule. If you have not had a chance to complete the paperwork, we request that you arrive 45 minutes early. If you need to cancel or reschedule, we would ask that you give us proper notice. We know your time is valuable and we will make every effort to accommodate your schedule.
Please remember when requesting a refill, we will need the name of the prescription, your name and date of birth and name and number of your preferred pharmacy. We will do our best to have all refills filled upon the same day of request, however, refills may take up to 24 hours. You can request your refill on our patient portal at any time.
